To receive one (1) entry without a purchase fill out the form. Complete name, email address, daytime phone number (with area code) and date of birth. Limit one alternative entry per person. If unable to contact the potential winner or a potential winner is not in compliance with these Official Rules, prize will be forfeited, and an alternative winner will be selected.
